What is the Confidential Information Form 11.1?

The Confidential Information Form 11.1 is another form required at the beginning of a divorce, custody, parenting time, or child support proceeding. It gives the court the social security numbers of the parties and the children involved.
This document, unlike most court documents, is part of the confidential court file; meaning, it is not available for public view on a public access court computer.
